A 55 yr old male with CKD on MHD

CHIEF COMPLAINT:
55 year old Male patient ,farmer by occupation Came to OPD with chief complaint of burning micturition since 2 days ,pedal edema since 2 months
HISTORY OF PRESENT ILLNESS:
Patient was apparently asymptomatic 6 months back then he developed CKD with pedal edema
3 dialysis completed till now
Complaints of burning sensation since 2 days
HISTORY OF PAST ILLNESS:
K/c/o Hypertension since 20 yrs on medication
( Nicardia 10mg-PO/OD)
Not a K/c/o Asthama,Diabetes,epilepsy,TB
PERSONAL HISTORY:
Appetite-normal
Diet-mixed
Bowel and bladder Movement-Regular
No allergies
Alcoholic occasionally
GENERAL EXAMINATION:
patient is Conscious,coherent and cooperative
-No pallor
-No icterus
-No lymphadenopathy
-No cyanosis
-No clubbing of fingers
VITALS:
Temperature-afebrile
Pulse rate -86 bpm
Respiration rate -23cpm
Bp-130/80 mm Hg
Spo -96%
SYSTEMIC EXAMINATION:

S1 & S2 sounds are heard

PROVISIONAL DIAGNOSIS:
CKD on MHD
TREATMENT:
T. NICARDIA 10mg PO BD
T. NODOSIS 500mg PO BD
T. LASIX 40 mg PO BD
T. OROFER XT PO OD
T. SHELCAL PO OD
Inj. Erythropoietin 4000 IU S/C weekly once
